A. If the use will be in the United States, its dependencies, or in Canada:

• Specific details about the work you wish to use, how it will be used, whether the use is a non-profit or educational use, and how you intend to publish and distribute the use once completed.
• If the intended use is in a performance, state the size of the venue, cost of admission, and whether any recording is planned (please note: only recording for archival purposes is permitted).
